There is no doubt that road safety and travel for children with disabilities and medical conditions, some of our most vulnerable road users, has reached a new level of awareness. Children are at greater risk of injury or death in the event of a car crash as their bodies are not able to withstand the forces in a crash. Even more vulnerable, are children with disabilities and medical conditions. There is still a long way to go, and many are at work to achieve greater awareness and lasting changes across many sectors in Australia.

&nbsp;

Promoting and facilitating safe road travel for children with disabilities requires a team approach. There are many considerations to address, and this goal can only be achieved when everyone works together.&nbsp; Both prescribers and suppliers of special purpose car seats should be highly knowledgeable in this area, with suppliers also having expert knowledge on products. For those entering this specialised field, support and training are recommended. Valuable resources for evidence-based information and training can be found at MACA, Mobility and Accessibility for Children in Australia website (check out <a href="https://www.macahub.org" style="color:#0563c1; text-decoration:underline">macahub.org</a>). &nbsp;Product training and education offered by APEX Mobility adds to a comprehensive plan to build expertise in your role for this area. &nbsp;&nbsp;&nbsp;

Clinicians should be following a best practice approach, which includes MACA&rsquo;s four step process: Assess, Plan, Implement and Review. Both clinicians and suppliers play essential roles in supporting solutions for safer car travel, whether for short-term or long-term needs.

Assess &amp; Plan:

Suppliers and product specialists can begin getting involved in this process by seeking to understand the specific needs of the child and their family. Reaching out to clinicians to discuss findings from their assessments (that have led them to consider a special purpose car seat) is a good way to start identifying suitable solutions and setting up for a successful trial. 

When considering a special purpose car seat, a crucial part of the planning process is completing a trial. Suppliers are well positioned to support clinicians and families to work through available options to find a suitable solution. Tips for completing trials with clinicians and families include:

<ul>
	<li style="margin-left:8px">Ensure you have an in-depth understanding of the manufacturer&rsquo;s instructions and the correct installation and use of the special purpose car seat.</li>
	<li style="margin-left:8px">Avoid future challenges by installing the special purpose car seat in the vehicle/s correctly for the trial. </li>
	<li style="margin-left:8px">Do not trial the seat uninstalled. Not only can this raise safety concerns during the trial as the base may be unstable, but it also means missing a vital step to assess the suitability of the special purpose car seat&rsquo;s attachment to the vehicle/s.</li>
	<li style="margin-left:8px">Provide accurate education on both the correct installation and safe use of the seat along with its features. </li>
	<li style="margin-bottom:11px; margin-left:8px">Keep an eye out for any inaccuracies when the seat is in use and continue to support the trial by getting family/carers involved, including extended family or carers that may be involved with the child&rsquo;s weekly travel routine. </li>
</ul>

Implement &amp; Review:

When preparing for the delivery of a special purpose car seat ensure you allow sufficient time to provide education and training. The aim should be to support the clinician, child, and family to achieve their travel plan goals through correct installation and safe use. They should feel confident in the overall set up and use of the product. 

<ul>
	<li>Aim to complete this step with all family/carers that will be a part of the child&rsquo;s weekly travel routine. </li>
	<li>Keep an eye out for any errors when observing the family/carers installing and using the seat and any features or options. Offer assistance and further training to eliminate these. </li>
	<li style="margin-bottom:11px">Offer to attend the review appointment or to return to check for growth and safe use. </li>
</ul>

Supporting safe road travel is a specialised area of practice for clinicians and suppliers which delivers vital services, products and information for children and families to achieve travel goals. Supporting safe road travel with special purpose car seats

Being able to pop into the shops or go for a walk around the local park should be easy, right? 

But what if your child has a disability, and their needs can&rsquo;t be met in a Baby Bunting special? Or they still need the support of a stroller to manage distances, but the regular strollers are now too small? These considerations can also extend to the young adult population, where a wheelchair with complex seating might meet their postural needs but doesn&rsquo;t fit with transporting in the family vehicle, their lifestyle, or the needs of all involved in the users&rsquo; care. Getting out and about can then turn into a mammoth task leaving some families not knowing where to start and at risk of becoming isolated at home without a suitable mobility solution. 

So where is a good place start? 

&nbsp;

Assessment is Key

As always, a thorough assessment with a therapist is a great first step in finding what best suits a user&rsquo;s mobility needs. This will gather the information required to be able to make a great decision for a suitable mobility device, whether this be for a wheelchair or stroller. 

Within this assessment, there are some key questions to answer about the user and their needs. &nbsp;

Postural Abilities and Tone Considerations 

<ul>
	<li style="margin-left:8px">What are the postural capabilities and challenges for the user? Determining the level of postural support required by the user will establish, to some extent, the range of strollers that could be suitable.&nbsp; Higher postural support needs mean more components on a stroller. This will impact how the stroller folds and its weight. </li>
	<li style="margin-bottom:11px; margin-left:8px">Is high or low muscle tone present? If a user also has challenges with spasticity or high tone, then the structural strength of the device will be a factor to consider.</li>
</ul>

Physical Biometrics

<ul>
	<li style="margin-left:8px">What are their joint range of movements including asymmetries and any non-reducible postures? This will also determine if seating supports are required in the stroller and if they can be comfortably accommodated. </li>
	<li style="margin-bottom:11px; margin-left:8px">What are the physical measurements of the user including limb lengths and trunk/hip width? What is the user weight and the weight of essential items they will be travelling with regularly? Here we are talking about the user&rsquo;s fit in the stroller and the combined weight we are asking a mobility device to handle on a regular basis. A stroller may indicate a certain weight maximum which includes not only the weight of the user, but all the other items carried for their use. Examples include toiletries, oxygen or suction machines for respiratory support, toys/games, and clothing. All of this may be essential to the outing, so consideration should be given to whether the proposed stroller has the capacity to carry it all. </li>
</ul>

Pressure Considerations 

<ul>
	<li style="margin-bottom:11px; margin-left:8px">Are there any specific pressure relieving needs including skin integrity issues and positional change requirements? If so, then there will need to be pressure relieving measures over and above general comfort levels in the seating of the stroller or even a consideration of whether a stroller is the best option.</li>
</ul>

Transfers

<ul>
	<li style="margin-bottom:11px; margin-left:8px">What are the user&rsquo;s transferring capabilities to access the stroller? If considering a three-wheeler, determine the ability of the user to safely step over the middle wheel to get into the stroller or if the carer will be assisting them. When considering a four wheeled stroller for a user performing standing transfers, the seat to floor height will be an important consideration for safely achieving this. Lastly, the stability of the stroller during this transfer would need to be assessed for suitability. </li>
</ul>

Managing Day-to-Day Tasks

<ul>
	<li style="margin-left:8px">What will be the use of the stroller? If a stroller is for all mobility and used every day, then considerations for seated comfort may require more priority. If there are positional changes required for comfortable use, then a more complex mobility option may be indicated. </li>
	<li style="margin-left:8px">Where does it need to take the user? Here it&rsquo;s the terrain in the intended environments of use that&rsquo;s important. &nbsp;Most strollers will run well over flat, hard surfaces but getting over grass, gravel, and even rough walking tracks is not recommended. For recreational use or environments with uneven and challenging terrain, a differently purposed set up is more appropriate. </li>
	<li style="margin-bottom:11px; margin-left:8px">How far will they be travelling? This is about the robustness of the mobility device and its fitness for purpose. A superlight, compact stroller may be great for short urban trips but is likely to be unsuited to regular hard use. </li>
</ul>

Transport

<ul>
	<li style="margin-bottom:11px; margin-left:8px">How will the device be transported and who will be transporting it? Some key things to think about here are where in the vehicle the mobility device needs to be stowed. Consider the dimensions of the mobility device when folded up for transport and how it folds, how easy it handles when folded and how heavy the device is to lift (especially for the most petite of the care team).&nbsp; </li>
</ul>

Sensory and other Needs

<ul>
	<li style="margin-bottom:11px; margin-left:8px">Are there sensory needs? Look closely at the accessories and options available with a stroller. These include trays, types of material liners, and even speed of foldup can all be factors that have an impact. </li>
</ul>

&nbsp;

All of these questions should come together to compile a detailed picture of what the physical requirements of the user are and a &ldquo;script&rdquo; of what is needed from a stroller in terms of features and terrain capabilities can be completed. This provides the best position to make an informed choice. Remember, a compromise may be necessary so keep in mind the most important requirements.

Function and efficiency are goals that are factored into the selection of a manual wheelchair frame, its material and components, whether folding or rigid. We also understand that is not just one part or one feature that achieves these goals, but the system as a whole. Here we will take a closer look at Motion Composites flagship design, the HELIO C2, a carbon fibre, folding manual wheelchair, and how it helps achieve these goals. 

Many reasons exist why a folding wheelchair frame could be the most appropriate set up for a user, including:

No matter the reasons why a manual folding frame is needed, frame features, components and adjustability will impact the user&rsquo;s function and the efficiency of how the system works as a whole. The HELIO C2 is one of the lightest, ultra-light folding wheelchairs available with a naked frame weight of just 4.3kg (16x16). We know that weight is a contributing factor in facilitating efficiency in a folding frame and it also allows ease of transportation when needed. Looking closely at the HELIO C2, we can see how it achieves this and how this along with other standout features serves as a basis to build an efficient system. &nbsp;This will also highlight how the HELIO C2 aims to achieve efficiency and reduce risk of injury to the user.

How does the HELIO C2 achieve function and efficiency from its form? 

Through the material of its build, the components of its naked frame and castor housing.

The HELIO C2&rsquo;s frame is manufactured using carbon fibre composite (image 3). This is created when carbon fibre fabric (image 2), made up of several thousand filaments (image 1) is mixed with a matrix of plastic resin. Although this produces a very light weight and strong material, additional manufacturing techniques are needed to ensure that the wheelchair maintains strength and durability in areas that need reinforcement. This is achieved in three ways:

	<li style="margin-left:8px">manufacturing technique &ndash; layering, wrapping, weaving and braiding (below image 1) and bladder molds (below image 2). </li>
	<li style="margin-left:8px">Multilayers and offset orientation of the carbon fibre fabric &ndash; each with a specific strength benefit (image 3 &amp; 4)</li>
	<li style="margin-bottom:11px; margin-left:8px">type and amount of resin used. </li>
</ol>

The engineers at Motion Composites continue to invest efforts to produce high-end manufacturing techniques to deliver high performing wheelchairs.

When looking more closely at carbon fibre, we can see some distinct qualities that gives it an advantage over other materials. Specifically, these are:

	<li style="margin-left:8px">High vibration dampening quality</li>
	<li style="margin-left:8px">No welding and a manufacturing technique that provides strength where it is needed</li>

	<li style="margin-bottom:11px; margin-left:8px">High corrosion resistance</li>

Each characteristic of this material and how it is manufactures has an impact on the efficiency of the frame with the user&rsquo;s experience and health front of mind.

Castor housing

Squaring of the castors is essential for proper tracking, decreasing castor flutter, and ultimately promoting propulsion efficiency. The infinite adjustability of the housing on the C2 frame allows for precision so that this can be achieved with ease. The unique design with the trailing arm also ensures increased strength and durability. 

Overall, when we look at some of the forces that steal away energy when propelling, such as movement within the frame, vibrations and torsion, we can see how the HELIO C2 is built to reduce these. In doing this, it provides a foundation whereby a folding manual wheelchair promotes health and reduces risk of injury during self-propelling when it is set up correctly for the user. &nbsp;&nbsp;

For those familiar with the song and dance, doing the twist will soon have another meaning. APEX Mobility and international supplier Klaxon are in the final preparation stages to launch the revolutionary power assist device, the Twist. The long-awaited power assist device (PAD) promises multiple avenues to achieve optimal access and occupational engagement for manual wheelchair users.

Researchers have completed extensive studies in propulsion efficiency and shoulder health for manual wheelchair users who self-propel. A full-time user can propel up to 2500 &ndash; 3500 repetitions daily and preserving shoulder health to maintain independence is paramount. Sustaining a shoulder injury of any nature, has significant potential to impact every aspect of daily life and level of independence. Consider being unable to lift your arms to wash your hair or put on a shirt, along with losing independent mobility.

Addressing the need to preserve shoulder health and function for manual wheelchair users that self-propel; clinicians and users work together to achieve efficiency in propelling with the slightest physical output for maximal distance. This is achieved through building an optimal system setup (through wheelchair frame, seating, and weight distribution, for some examples), education and training on manual wheelchair skills, and introducing a power assist device.

Up to now, we have had multiple PAD options that offer one type of setup, rear mount, hand rim activated, joystick control, or front mount with handlebar. Selecting the most appropriate one typically depends on which PAD meets most goals. This is where the Klaxon Twist will introduce the ability to offer multiple setups, opening up more possibilities, and optimise and maintain occupational engagement.

Rear mount power devices, especially ones like the Twist that tuck right under the frame, provide partial assistance and are an excellent option for indoor environments. The only limiting factor with these is that the front castors remain loaded and unable to navigate over small lips and terrains that may be a little rough and bumpy; here is where front mount devices would be considered. The standout benefit of the Twist is the option to have both rear and front mount (handlebar or hands-free) on the device. This opens up access to multiple environments where one device can provide benefits in optimising occupation engagement and preserving the user&#39;s overall physical and mental well-being.

On a final note, funding bodies generally approve one PAD for a rider, and the need to get it right is life changing. Careful evaluations and discussions between clinicians and users will continue to identify the most appropriate assistive technology to meet goals. With the right application, the Twist has the potential to really optimise someone&#39;s life outcomes and engagement.

Therapists and end users look to providers of assistive technology for guidance on how and which products will ultimately help achieve goals for engagement in daily life. Complex assistive technology becomes a part of daily life and impacts how the end user and/or their family functions in their environment. It is for this reason that our responsibility to understand the products is so important. During the pre-trial and trail phase, knowledge regarding the features, limitations and full potential of products is shared with the therapist, person, family and or carers. This often leads to essential open discussions that guides the final selection that best suits the person and facilitate their goals. 

As such, a high level of product knowledge is essential and plays a significant role in building relationships with all parties. A strong working relationship is based on trust, reliability, honesty, and competence (and of course open and effective communication along the way). 

Competence: Ensure you have the experience, skills and knowledge to script, fit and follow up provision of complex assistive technology. This also allows you to provide reliable information and education. Gain skills and knowledge by continually engaging in training, education, and networking with those in the industry to keep up to date for best practice. 

Honesty: Be open and truthful, if you don&rsquo;t have all the knowledge and/or skills, seek ways to find this by attending days like APEXPO and provide timely feedback if questions are asked during a trial that need further investigation. If one product cannot meet all the needs (as many do), be open and honest about the assistive technology to help set up expectation and work collaboratively. 

Reliability: This includes not only doing what you say you will do, but also having the knowledge to educate and inform so that the assistive technology products do what you have said they will do. It also includes being there to follow up after delivery to continue to provide support.

Trust: Building trust bridges a strong connection with others and has a significant impact on future interactions and opportunities to continue working together. Trust cannot be achieved without competence, honesty, and reliability. 

So what does this have to do with APEXPO? The day set aside for APEXPO 2023 is our opportunity to provide those attending with knowledge and skills that will build or consolidate competence in products, allow you to be open and honest with manufactures and suppliers to ask questions and bridge gaps in knowledge or skills. The goal is to ensure you have the level of knowledge and skills to build effective relationships with therapists and end users and all of which is aimed at instilling trust in our products and our people.

The&nbsp;Thomashilfen Commander&nbsp;is a highly adaptive positioning car seat for children with special needs.&nbsp;Especially for children with little upper body control, the fixed lateral trunk supports offer this extra support.&nbsp; 
 
The&nbsp;Defender&nbsp;allows children with special needs 360&deg; of impact protection. The head, trunk and thighs supports, combined with&nbsp;unique accessories that offer many positioning options. The seat allows for growth while remaining narrow enough for a second car seat as well as another child to fit in the back seat.

Our distribution centres were briefly closed for dispatch from 30 June to 4 July while we completed a stocktake of all our warehouses. We’ve adjusted our variance reports and we’re ready and raring to start the new financial year.

Welcome to this month&rsquo;s edition of the APEX Mobility dealer newsletter. &nbsp;Coming right off the back from a successful APEXPO-2022 and ATSA Sydney, we are also looking forward to Source Kids Melbourne next weekend (18-19 June). 
 
In this month&rsquo;s edition, we will be discussing current and upcoming APEX Mobility marketing campaigns, pricing updates, and three products in focus: the Madita Fun high-low indoor chair from Schuchmann, the new Klaxon Twist, and changes to the Kidoo Positioning Chair.

PRICING UPDATE

Firstly as to pricing. &nbsp;APEX Mobility group our costs into: (a) equipment purchases from factories around the world, (b) the cost to get that equipment to and into Australia, and (c) the cost to run our Australian operation (wages, fuel, rent, etc). &nbsp;As being the TGA Australian Sponsor for over 24 brands, we typically see a price rise from one or two manufacturers per month. &nbsp;In the last few months, especially since the Russian invasion of Ukraine, we have seen a dramatic increase. &nbsp;This seems to be because of the negative impact this war has had on raw materials (steel, plastic, timber, etc), as well as adding to the pressure on global shipping and labour costs. &nbsp;Additionally, the cost of living continues to increase globally, and living here in Australia we are definitely not exempt. &nbsp;Inflation continues at over 5%, and import costs are higher than ever before. We have reviewed these costs in conjunction with manufacturer price increases on individual brands, and will be passing individual price rises of between 4% and 11% across most of our brands,&nbsp;effective 1 August 2022. We are giving our dealers 6 weeks&rsquo; notice of these increases, and will be working hard between now and August to ensure all our price lists and script forms are updated to reflect the price increases before they come into effect.

Please note that there has been&nbsp;an update to the design of the&nbsp;Kidoo Positioning Chair&nbsp;backrest that impacts the mounting of the lateral supports. The updated design does not have the mounting slots on the side of the frame which means that the lateral supports now mount to the front of the backrest plate.&nbsp;

Our second Distribution Centre is now open! Our Melbourne warehouse has moved and upgraded. Now located in Truganina, it features a state-of-the-art warehouse facility allowing us to stock a large number of our Quick Ship product lines, including Starfish, Rifton and xRover among others. This will be especially useful for our dealers located in Victoria, as it will mean faster shipping times!

The new location also features an Experience Centre, where clinical and product training and other events will be held. Contact your APEX Mobility account manager to arrange a visit, and stay tuned for exciting events planned here in the coming months!

To keep our equipment in front of our customers and referrers, APEX Mobility sponsored a major AT exhibition in April 2021, where Therapists and Equipment Users logged in and viewed a huge range of equipment demonstrations and were able to ask questions.&nbsp;

New recreational strollers added to our range!

Our new National Distribution Centre is now open! Located in Ingleburn, the larger space gives us the opportunity to hold much more &lsquo;quick ship&rsquo; inventory than previous, and streamline our warehousing processes, meaning we can deliver equipment to our valued dealers more quickly and efficiently.&nbsp;

We have also received several container loads of assistive technology equipment from around the world including Rifton, Ormesa and Klaxon, which is nicely filling up the new space!
